What it catches

  • Vague outcomes ("will improve communication" — by when? measured how?)
  • Provision without specifics ("access to OT" — how often, for how long?)
  • Missing sensory accommodations even though sensory needs are described
  • Outdated diagnoses or assessments older than statutory limits
  • Targets that aren't SMART (specific, measurable, achievable, relevant, time-bound)
  • Disconnects between what the parent reports and what the document says
